Partners in Caring

Partners in Caring is a collaborative dedicated to serving individuals sixty years and above and individuals with disabilities. The goal of the program is to provide a broad range of supportive services, assisting them in their desire to age in place and live as safely and independently as possible. Partners in Caring agencies consist of American Red Cross of Central NJ; Enable, Inc., Greater Mercer TMA –Ride Provide,Jewish Family & Children’s Services of Greater Mercer County,Princeton Senior Resource Center, and Contact of Mercer County, NJ, Inc., of which Reassurance Contact and Retired Senior and Volunteer Program (RSVP) are programs.

The collaborative provides services for seniors and people with disabilities in northern Mercer County include, but are not limited to: case management, information and referral, counseling, home-delivered meals, caregiver support, accessibility services, transportation, and volunteer support guaranteeing that clients involved or enrolled with one agency automatically have the availability of the services of all the collaborative partners. For further details call Davida Nugiel 609-987-8100 or email davidan@jfcsonline.org.

Project Connect

Project Connect is a collaboration of eight local social service agencies created to aid individuals sixty years and above, and people with disabilities who are experiencing age-related difficulties.  The program is open to those residing  in the greater Trenton area.(Trenton, Ewing, Lawrenceville and parts of Hamilton).  The goal of Project Connect is to provide case management services to promote health, dignity and independence to older adults and individuals with disabilities.

Services provided through Project Connect include, but are not limited to: care coordination, home health care, mental health and substance abuse services, transportation, socialization and recreation, home delivered meals, and individualized volunteer support.

Participating agencies include Jewish Family and Children’s Service of Greater Mercer County Mercer Street Friends, providing Catholic Charities, Mount Carmel Guild, Enable Inc., The ARC Mercer, Mobile Meals of Trenton Ewing, and Interfaith Caregivers.

The key to this collaborative project is that clients will no longer have to wait for needed services, be put on waiting lists, get turned away for lack of eligibility or resources, or ‘fall through the cracks.’ These eight agencies will ‘wrap’ services around clients, prioritize their needs, and provide care coordination and case management that will allow clients to live with dignity in their own homes whenever possible. It will also help avoid premature institutionalization.

To learn more about how Project Connect can assist senior citizens please contact Ellen Schwartz at 609-987-8100 or email ellens@jfcsonline.org.

Partners in Caring and Project Connect are both funded in part by a grant from
